Note: We accept most major insurance plans. Pre-authorization may be required for many of the treatments administered by LAHOMG. If you are unsure about your coverage, please call our office and we will be happy to assist you.

If you are not covered by health insurance for your visit or any of the treatments we provide, we may be able to assist you with costs or make special financial arrangements for our services. Please call our office and we will be happy to assist you.

• Please allow 1-2 hours for your first visit.
• Arrive at least 30 minutes early to allow time for completion of paperwork and verification of coverage or pre-authorizations.
• Bring any records, x-rays, or physicians' referrals
• Bring list of medications you are currently taking
